Primary Care Nurse Practitioner

As a Primary Care Provider, you will provide primary care to patients in long-term care facilities.

This includes:

Developing and implementing personalized treatment plans based on patient needs Offering medication management, including prescribing and monitoring medications Collaborating with a multidisciplinary team of healthcare professionals to ensure patient care Maintaining accurate medical documentation in line with regulatory standards Providing education and support to patients and their families regarding mental health conditions and treatments Qualifications:

Current Nurse Practitioner license in specified location At least two years of experience is required Experience providing care to geriatric populations in long-term care settings is preferred but not required Excellent communication skills, especially when working with facility staff and residents' families Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs
