Where You Will Work:

Our nurse navigators provide longitudinal care through clinical assessment and triage, imaging review, risk stratification, symptom escalation, patient education, and care coordination. Much of this care is delivered by telephone or virtually, in close partnership with pulmonary providers, radiology, thoracic surgery, oncology, primary care, and operational support teams.

This is a hybrid position that includes remote responsibilities and in-person work at Kenilworth Medical Office Building, with periodic travel to ambulatory practices for education, outreach, and program support.

We’re Looking For:

We are seeking a dedicated and experienced Registered Nurse (RN) to join our Lung Nodule Program. The ideal candidate is highly organized, adaptable, comfortable working independently in a hybrid environment, and skilled at collaborating across multiple disciplines. Strong communication, clinical judgment, patient education, and care coordination skills are important for this role. Experience in pulmonary medicine, oncology, nurse navigation, ambulatory care, or care coordination is preferred.

What You’ll Do:

The RN will serve as a nurse navigator for patients with identified lung nodules and will:

Assess clinical information and imaging recommendations to help determine appropriate next steps. Perform clinical triage, risk stratification, and symptom escalation. Provide patient outreach, education, and support by telephone, virtually, and in person as appropriate. Communicate with patients, referring providers, and interdisciplinary care teams. Coordinate pulmonary access, appointments, diagnostic follow-up, and multidisciplinary review when indicated. Identify and help resolve barriers to care while monitoring patients through the recommended pathway. Support standardized workflows, tracking processes, and quality-improvement efforts that promote timely follow-up and reduce gaps in care. Provide education and outreach to ambulatory practices regarding the lung nodule pathway.

Requirements:

Completion of an accredited or approved nursing program. Current Registered Nurse license issued by the state in which the teammate practices. Current BLS certification. Minimum of one year of nursing experience preferred. Pulmonary, oncology, nurse navigation, ambulatory care, or care coordination experience preferred.
