In this field-based role, you’ll conduct scheduled in-home Well-Child Visits, help identify health needs early, close care gaps, and keep kids connected to appropriate follow-up care.

What You’ll Do

Conduct scheduled in-home Well-Child Visits throughout Maricopa County Review pediatric medical history, risk factors, vitals, and appropriate screening information Complete preventive and age-appropriate pediatric assessments Document visit findings accurately in the EMR Educate patients and families on recommended next steps Identify care gaps and coordinate appropriate follow-up Manage your local route without reporting to a clinic each day

Must-Haves

Active Nurse Practitioner license in Arizona Registered/enrolled with Arizona Medicaid PNP-PC certification, or another NP certification appropriate to provide pediatric services within your population focus, education, national certification, and demonstrated competency Valid driver’s license and reliable transportation Comfortable working independently in patients’ homes
