Summary

The primary responsibilities are to develop and maintain relationships with community-based organizations, schools/colleges, healthcare providers, local agencies, municipalities, and other key partners in the grant's catchment area.

This position will represent the Tobacco Treatment Program at community events, health fairs, meetings, educational programs, other public events, and coalition activities. Focus will be on the Vape-Free Schools initiative, Tobacco-Free Multi-Unit Housing and Worksite policy, Young Lungs at Play, YRBSS, and Clean Indoor Air Initiatives including requested site visits.

This will include targeted outreach to youth, disparate populations, behavioral health organizations, and chronic disease integrations as outlined in the goals set by the American Lung Association Tobacco Grant. Applicable activities must be documented and reported though American Lung Association and PHMC portals. There may be additional responsibilities as grant requirements evolve, and as applicable to this position and outreach needs.

Job Duties

• Conducts outreach in-network and outside provider practices to establish cessation partnerships.

• Create and maintain partnerships with community-based organizations, including but not limited to: schools/colleges, local agencies, municipalities, and other key partners.

• Document and report outreach activities through appropriate American Lung Association and PHMC portals.

• Represent the Tobacco Treatment Program at community events, health fairs, meetings, educational program, other public events, or coalition activities • Identify and partner with schools, multi-unit housing sites, worksites, public municipalities, public outdoor spaces, and other sites covered by the Clean Indoor Air Act to improve tobacco-free policies

• Carries out necessary tasks for triage and data collection.

Minimum Qualifications

Education - Required • High School Diploma/GED Experience - Required • 5 years managing large projects and budgets and • 5 years experience public relations. Knowledge, Skills and Abilities - Required • Knowledge of Microsoft Word, Access, and Excel. • Experience managing grants. • Public speaking experience. Licenses and Certifications - Required • DL - Driver's License_PA - State of Pennsylvania Upon Hire and • CTTS - Certified Tobacco Treatment Specialist - Granted by Recognized Issuing Authority Within 1 Year Preferred Qualifications

Education - Preferred • Completion of a tobacco treatment program through one or more recognized tobacco treatment centers that are ATTUD accredited. Experience - Preferred • 1 year of experience with tobacco cessation program activities or related counseling (addiction). Additional Qualifications

Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
